Procedure & Form

Following document are required to get admission in DHMS at The HMC Gujranwala:

1. 10 Pics (Passport Size)
2. All Academic Documents 2 attested Copies each
3. Matric/FA/BA/etc. Certificate 2 attested Copies each
4. CNIC of Candidate/Father/Guardian 2 attested Copies
5. Admission Form duly filled and signed by the candidate.
